I am available to complete ADHD evaluations for children ages 6-12. The child does not have to be in my therapy practice. However, my scope of practice limitations do apply to both therapy and evaluations, so please check the scope page here before contacting me about an evaluation. If there are no issues beyond my scope, please start by emailing me a few details about why you’re seeking ADHD evaluation and what symptoms you’re seeing. All evaluation meetings are done by telehealth (zoom). 

Please note, my evaluation only screens for ADHD, and is not a comprehensive psychoeducational or neuropsychological assessment. The fee for my evaluation is $1,200 and is paid to me directly. I don’t take insurance, nor do I get involved in any way with insurance companies. I will provide a receipt that includes relevant insurance codes that you can submit for reimbursement if you like. 

The evaluation includes the following: 

• Parent interview

• Child interview (with parent present)

• Mailing/completion/scoring of the Conners 3 Parent, Teacher, and child (if 8 or older) questionnaires (short form) 

• Teacher consultation (if needed to clarify teacher ratings)

• Parent summary meeting (30-mInute)

I will provide you with a 1-2 page summary with my diagnostic findings, which we will discuss in the summary meeting. 

After the summary meeting, I will send a Venmo or Paypal request for the fee and will provide a receipt with relevant reimbursements codes.
